Services and Policies
- Seg Heights: PLEASE FOLLOW THESE INSTRUCTIONS IN FIGURING SEG. HEIGHTS ON ALL LENSES: S. H. is the seg. height as figured by the box system. We use the box system for all our calculations and ask you to take this into consideration when prescribing.
- Frame to Follow: Please indicate patient's name, your name and invoice number when sending the frame.
- Executive P.D.'s: Executive lenses are monocentric, therefore, both near and far P.D.'s cannot be obtained. Today's large frame sizes have greatly increased the problem. When we have to grind prism for decentration we cannot control upper and lower centers independently. Unless you specify otherwise, our policy will be to grind prism in the distance to get distance P.D.
- Lab Do-Over: Please return original invoice with lens or lenses to be corrected and a note describing reason for return. If no lenses are returned with request, the Rx will be processed at full charge.
- Doctor Refractive Error: See warranty below.
- Telephone Rx Errors: Rx's are read back to the person ordering. If you do not wish to have the Rx repeated to you, it is implied that the information given to our Service Representative is correct, and no credit will be given for incorrect Rx's.
- Cancelled Lenses: Cancelled Rx's will be billed if lenses have already been ground.
- Old Frames: We are not responsible for old frames sent in with an Rx. We will notify the account of any frame breakage or damage, and, if needed, will find a replacement at a reasonable price.
- Scratch Warranty: Finished Products purchased with antiscratch coating are warranted against scratching for 365 days.
- Quoted Prices: Phone quotes of prices are given for your benefit. Rx's made from those quotes may contain additional billable information not quoted, and will be billed as the presented Rx.
Prescription Optical Finished Rx Warranty
365 Day Warranty Policy*
We will remake any finished prescription at no charge due to refractive change, non-adapt, measure change, or patient dissatisfaction for a period of 365 days from original order date. Lenses ordered with anti-scratch coating will be warranted against scratches for 365 days.
Progressive Warranty*
If adjustment cannot be made by patient to his/her progressive lenses within 180 days, we will make a pair of Bifocals or 7x25 or 7x28 Trifocals at no charge.
Drill Mount Warranty*
Trivex material and Resolution lenses will be warranted against breakage for 1 year from date of invoice. All other drilled materials will be warranted for 90 days.
*Remake offer good once per prescription; A copy of the original prescription order and the original
lenses must accompany all warranty requests; Lenses only not covered; Further processing of lenses (tinting,
roll & polish, AR coating, bleaching, etc.) after they leave Prescription Optical voids this warranty.
